How do I create a contact form?

    1.    Log into your site.

    2.    Add the Form element to your site.

    3.    Roll over the Form tool bar and click Edit Form from the dropdown.



    4.    You will be brought to a new form page where you can begin adding fields to your form.



    5.    To label your new form click on the title Untitled form.



    6.    In the Form Name field type in the title of your form.  For this example I will make a form for a Contact Us
page.



    7.    If you would like to receive a notification each time a form is filled out, click the Email New Entries box
and type in your email address.



    8.    Click on the Add a field tab to begin adding fields to your Contact Us form.



    9.    Click on the Name, Address, Phone and Email buttons to add those fields to your form.  You can add any other field you like to the list.



    10.    Your form will now look like this.



    11.    Click the Save button.
